1. Estriol is preferred as there is thought to be less systemic absorption and low activity with respect to stimulation of breast and endometrium.
2. Low potency vaginal estradiol and estriol preparations vary in their licensed use. They may be used unopposed for symptoms of urogenital atrophy only at the recommended dosage interval. Higher frequency use may require progestogenic opposition if endometrial tissue is present. These vaginal preparations can be safely added to opposed but low dose systemic regimens if urogenital symptoms persist. Risks from repeated initiation courses may be higher than low dose maintenance. Vaginal conjugated oestrogen cream is a systemic high potency preparation that requires opposition.
